Completion of membership or joining forms

Full name (first and last), ID number, gender, email address, mobile number, marital status, date of birth, country of birth, residential address, designated contact Persons (full name; relationship to the user; mobile number; date of birth; ID number of the contact person), daily framework (age range; the framework you are in today; additional details about the framework), Physical disabilities (upper limb; vision; hearing), mobility, percentage of disability, diagnosis as having an intellectual disability/autism, status regarding recognition by: the welfare office, social security or the ministry of defense; upload a medical information summary form.

Applications for assistance grants (including medical treatments and rehabilitation equipment)
If you are already registered in the ILAN internal system, you only need to provide your ID number and date of birth for identification.
If you have not registered before, you must provide the detailed data in the section completion of membership or joining forms.

Applications for the Bergman Fund and Jewish Agency Scholarship

Storage and collection of personal information

The Association collects and stores information received from you as part of your use of the site. The Association will not have access to personal information about you other than the information you provide to the Association directly when entering your details on the site.

We receive information from PayPal and BIT about the person who made the donation and the amount donated, so that we can send you a thank you letter and receipt for making the donation.
